Treatment of Morbid Obesity
According to research diet, drug therapy and other non-surgical treatments have only limited success in treating morbid obesity. Surgery is the one sure option for reducing weight. When all other systems and treatments fail to control obesity surgery is recommended to avoid obesity’s adverse effects.
Surgical treatments offered are Gastric bypass and Adjustable gastric banding. These surgeries can be performed by laparoscopy.
Roux-en-Y Gastric Bypass
Refashions the stomach into a small pouch and bypasses a part of the small intestine. This results in a feeling of fullness with a small meal and also reduces absorption. This treatment has been tried and tested over the years. The procedure is irreversible and patients are advised to evaluate its risk vs benefits factors before surgery. Patients need to undergo a great deal of adjusting right from the size of their meal to the frequency.
Rapid weight loss is experienced for the first 3 to 6 months after surgery with a gradual weight loss upto 12-18 months after surgery. 70-75% of excess weight is lost.
Lap-Band Surgery or Adjustable Gastric Banding
This is a less extreme alternative. Here a balloon-like device banded around the stomach produces a small upper gastric pouch which causes an early feeling of fullness and satiety after a small meal. It is inserted through a small incision and then inflated to limit the capacity of the stomach. The procedure takes about 1 hour and requires 1 day hospitalization. The band can be tightened or loosened as required later without further surgery. It can also be completely removed to reverse the procedure.
Weight loss here is gradual with patients losing upto 55% of their excess weight within 18 months.
